Building a Digital Health Record Platform for a Network of Lagos Clinics
A network of 5 clinics operating across Lagos was managing patient records through a combination of paper files and disparate desktop-based software that could not share data between locations. A patient who visited one clinic could not have their records accessed at another. Lab results were delivered on paper and had to be manually filed. Prescriptions were handwritten. The lack of unified patient data was leading to duplicated tests, delayed diagnoses, and frustrated patients.
We built a cloud-based Electronic Health Record platform that unified all 5 clinics under a single patient database. The platform features comprehensive patient records, lab result integration, e-prescriptions, telemedicine scheduling, and role-based access control—all built to NDPR compliance standards. The system now serves 25,000+ patient records and has helped reduce diagnosis time by 40%.
| Metric | Result |
|---|---|
| Clinics Connected | 5 clinics across Lagos |
| Patient Records | 25,000+ digital records |
| Faster Diagnosis | 40% reduction in diagnosis time |
| Rollout Timeline | 6 months from kickoff to full deployment |
The Challenge
Fragmented Patient Data Across Five Locations
Each clinic operated independently with its own paper-based filing system. When a patient visited a different clinic location, the clinician had to rely on the patient's memory for their medical history. Lab tests ordered at one clinic were often repeated at another because the results could not be shared. The lack of centralised records made it difficult to track chronic conditions, medication histories, and treatment outcomes.
The clinics' existing IT infrastructure consisted of outdated desktop software installed on individual computers. There was no network connectivity between systems, no backup strategy, and no security controls for protecting patient data. A hardware failure could result in permanent loss of patient records.
Regulatory and Security Requirements
Handling health data in Nigeria requires compliance with the Nigeria Data Protection Regulation. Patient data must be encrypted, access must be audited, and patients have rights to access, correct, and request deletion of their data. The clinics had none of these controls in place and were exposed to significant regulatory and reputational risk.
Our Solution
Unified Cloud-Based EHR Platform
We built a comprehensive EHR platform hosted on AWS with encryption at rest (AES-256) and in transit (TLS 1.3). The platform provides a unified view of each patient across all clinic locations. Clinicians can access patient demographics, medical history, allergies, medications, lab results, visit notes, and treatment plans from any clinic.
The system supports structured clinical documentation including SOAP notes, diagnosis coding (ICD-11), medication prescribing with dosage calculations, and configurable clinical templates for different specialties. Role-based access ensures that only authorised personnel can view or modify patient records, with a complete audit trail of every access and change.
Lab Integration and E-Prescriptions
We integrated the platform with the clinics' lab systems using HL7 FHIR standards. Lab orders are created within the patient record, sent electronically to the lab, and results are automatically ingested into the patient's chart when completed. Configurable alert rules flag critical values that require immediate clinician attention.
The e-prescription module allows clinicians to generate prescriptions electronically, which are sent directly to the clinic's pharmacy or printed for the patient. The system checks for drug interactions and allergies at the point of prescribing, reducing medication errors.
Telemedicine Scheduling and NDPR Compliance
We added a telemedicine scheduling module that allows patients to book virtual consultations through a patient portal. Consultations are conducted via integrated video, and the consultation notes are automatically appended to the patient record. The patient portal also allows patients to view their medical records, request prescription refills, and communicate securely with their care team.
The entire platform was built with NDPR compliance by design, featuring granular consent management, data minimisation, automated data retention policies, patient data export and erasure capabilities, and breach notification workflows.
The Results
The platform was rolled out across all 5 clinics over 6 months, with 2 months of phased deployment followed by full go-live. Clinicians reported that having instant access to complete patient history reduced diagnosis time by 40% and eliminated unnecessary duplicate tests. The unified record system also improved care coordination for patients with chronic conditions who visited multiple clinics.
The e-prescription module reduced medication errors by 85% through built-in drug interaction checks. The lab integration eliminated lost paper results and reduced the turnaround time for lab results from days to hours. Patient satisfaction scores improved significantly, with the patient portal and telemedicine capabilities receiving particularly positive feedback.
Key Takeaways
- Unified records improve clinical outcomes. When clinicians have access to complete patient history, they make better decisions faster.
- Integration eliminates manual work. Lab and pharmacy integration removed hours of daily data entry and filing work for clinic staff.
- Security must be built in, not bolted on. Encrypting data and implementing role-based access from the start avoided costly retrofitting later.
- Change management is as important as technology. Training clinicians to adopt digital workflows was the most challenging part of the project.
